Hours of Operation

Office hours are 8:00 A.M. - 5:00 P.M. Monday through Friday; service available seven days a week depending on individual need(s).

Services Provided

Medicaid Service Coordination, State funding for environmental modifications, adaptive equipment, special needs and staffing supports to provide relief to in-home care givers. Services also include opportunities for recreation and training in a variety of daily living skills.

Also, weekend Respite Services at Agency operated Respite Home. Provide 24 hour staffing supports and recreational/socialization opportunities in the community.

Service Eligibility

Families who have a developmentally disabled family member or developmentally disabled independent client.

Referral from DDSO, telephone agency directly

Fee Schedule

None
Comments: Some services require Medicaid funding. Respite Home services are based on sliding fee schedule or direct Medicaid billing for qualifiers.
