Services Provided

Comprehensive diagnostic assessment; education and support for care giving families; adult day program; provides in-service programming; case management.

People suffering from memory impairment, Alzheimer's Disease, or a related disorder

Referral Procedure

Referrals accepted from physicians, public health and social service agencies, discharge planners, family, and friends.
