Services Provided

Services for people with mental illnesses including counseling; housing for people with mental illnesses; medication management

Certified residential services for adults with mental illness including supervised living, supportive apartments and supported housing. In addition is a newly created community residence which is devoted to alcohol-related concerns.

Service Eligibility

Previously hospitalized emotionally disabled adults ages 18 and older

Referral Procedure

Referrals from self or outside agency (St. Lawrence Psychiatric Center, CV/PH Medical Center, Crisis Center, etc.)

Fee Schedule

Set: Congregate Care Level 2 fee (Public Assistance, SSI, VA, etc)
Comments: Private pay is determined on sliding scale based on ability to pay.

Additional Information

Residences are located in various locations within the City of Plattsburgh. Programming fosters independent living skills, responsibility, and decision-making; length of stay is from six months/two years or more.
