Services Provided

Case Management services which provide family support through visits to the home, school, and other human service agencies to families residing in Franklin County for the MR and DD population.

Clinic services provided include primary, optometry, foot care, psychiatric and counseling.

Agency also provides:
Habilitation Services for the MR/DD population
Consumer directed personal care services
Care at home service coordination

Service Eligibility

Any individual with cerebral palsy or other developmental disability for case management and residential. General public for other services.

Referrals can be made by the family, family physician, or other human service agency worker; call; walk in

Set: Fee schedule available from agency; variety of insurances acceptable including Medicaid and Medicare

Volunteer Opportunities

Available dependent upon the area of volunteer interest and agency need. Internships also possible.
