Services Provided
The Intensive Case Management (ICM) program provides services to twelve children and youth who meet the Serious Emotional Disturbance (SED) criteria. Assistance is provided to children and their families to access services in the community with the goal of decreasing incidences of psychiatric hospitalization and out of home placement, improving functioning within the home, school and community at large and helping to empower families and children who participate in the program. Recipients are required to receive between two and four monthly face-to-face contacts depending on their level of need.

Individuals must be Franklin County residents and must meet SED eligibility
Referral Procedure
Individuals must go through the Single Point of Access process and all referrals should go to the Program Coordinator at 481-5746.

Volunteer Opportunities
Additional Information
This program accepts all referrals regardless of insurance type, serves Franklin County residents and is a voluntary service. Saranac Lake residents living in Essex County can also be serviced by this program. Bloomingdale and Lake Placid residents need to be referred to Essex County programs.
