Services Provided

The Supportive Case Management (SCM) program provides services to twenty adults who meet the Sever and Persistent Mental Illness (SPMI) criteria. Assistance is provided to individuals to access services in the community with the goal of decreasing incidences of psychiatric hospitalization, improving functioning within the home and community, and helping to empower recipients who participate in the program. Recipients are required to receive two monthly face-to-face contacts.

Individuals must be Franklin County residents and must meet SPMI eligibility

Referral Procedure

Individuals must go through the Single Point of Access process and all referrals should go to the Program Coordinator at 481-5746.

Comments: None, if available Medicaid is billed

Volunteer Opportunities

Additional Information

This program accepts all referrals regardless of insurance type, serves Franklin County residents and is a voluntary service. Saranac Lake residents living in Essex County can also be serviced by this program. Bloomingdale and Lake Placid residents need to be referred to Essex County programs.
